All employees can participate and enjoy the free wellness platform, tools, and programs available through Personify Health.

You can also save up to $360 per year on your medical insurance premiums as an individual.
Save an additional $60 per year if your eligible spouse/partner participates!
Only spouses and partners enrolled in a Boulder County medical plan can participate in the wellness program.

How It Works

Step 1

Complete the online Health Check Survey and Biometric Screening.

Step 2

Complete the Nicotine-Free Agreement OR Tobacco Journey.

Step 3

Achieve 5,000 wellness points through a variety of activities in the Personify Health app.

Once you complete step 1 and step 2,

Employees Enrolled in a Boulder County Medical Plan

You've earned $10 per month off your medical premiums!
You can start earning up to $200 in gift cards in 2026.

Employees Not Enrolled in a Boulder County Medical Plan

You can start earning up to $200 in gift cards in 2026.

Spouses and Partners Enrolled in a Boulder County Medical Plan

You've earned $60 per year off your medical premiums!
Spouses and partners are not eligible for prizes or gift cards.

Important Timelines

Dec. 15, 2025

This is the deadline to complete all steps for your 2026 medical premium discounts and the ability to earn gift cards in 2026.

Plan Ahead

We highly recommend completing your biometric screening well in advance of the due date.

This gives time to ensure your results are uploaded to the platform and you have ample time to complete Step 3. Results can take a minimum of 7–10 business days to be uploaded into the platform.
